INSTRUCTIONS FOR OFFICE OF CONSUMER AFFAIRS
INTERNSHIP APPLICANTS
Thank you for your interest in an internship with the Office of Consumer Affairs. Here is a checklist of Departmental requirements for all interns. These documents must be submitted and processed before the start of your internship.
o Application for Federal Employment (SF171) or Optional Application for Federal Employment (SF612) -- Type this form and complete everything but the signature and the date. Keep the original and make copies of the completed form. This will prevent you from having to complete the entire form again should you apply for another Federal government position later. The copy you submit should contain an original signature and date. The SF171 is only available in hardcopy. Please request a copy directly from the Office of Consumer Affairs(see e-mail address or phone number below).
o Student Volunteer Service Agreement -- Both you and your school sponsor need to supply the information and sign the Agreement.
o Student Volunteer Assignment -- This spells out what you will be doing while here at the Office of Consumer Affairs.
o Special Agreement Checks (SAC) -- This form is used for investigative purposes. This form is only available in hardcopy. Please request a copy directly from the Office of Consumer Affairs(see e-mail address or phone number below).
o Copy of Current Transcript or Proof of Enrollment Status
